Retiree Medical Forms
In an October 25 letter, all retirees were told that to change their insurance, they needed to submit a “2021 Transfer Election Form” AND an application form for the new insurance. The deadline for forms is November 30 at the RCSD Benefits office. Forms can be found below:
- 2021 Transfer Election Form (You must fill out this form and plan you are changing to below) If you want to move from Medicare Blue Choice or MVP to the Basic or the Retiree+ Enhanced EPO plan, you also have to complete the appropriate disenrollment form. If you want to move between the Medicare Blue Choice to MVP, the District needs the transfer election form and the application.
